What matters to me

Think about your main concern, what you hope to understand and anything about your circumstances that the team should know.

Questions to consider

Understanding my surgery

  • What should I understand about the benefits, risks and other options?

Getting ready

  • What are my instructions for medicines, eating and drinking?

Recovery and support at home

  • What help will I need, for how long, and what if I live alone?

Follow-up and getting help

  • Who do I contact during or after hours, and when is urgent help needed?

During the conversation

Take notes, ask for unfamiliar words to be explained, and check anything you are unsure about. You can ask for an interpreter, accessible information or a support person.

Before finishing, confirm your next steps and who to contact with further questions.

Print it and write down your priorities, your questions and the plan you agree with your team.
